Publication of Details: Names, contact details, and affiliations of participants will not be revealed during the survey.  Comments in a Delphi survey are not attributed to their author.  So while I intend to acknowledge participants in the final report, the acknowledgement will include name and affiliation only, your name will not be linked to your comments in this survey.  If you prefer your name and affiliation to be omitted from the final report you may request that at any time up to the release of the report.  You are also free to withdraw from this survey at any time, and at your request any comments you have contributed to the survey, that have not yet been disseminated to the group, will be deleted.

Names and other personal details will not be used for any purpose other than those described in the survey documentation.

The Human Research Ethics Committee at the University of Western Australia requires that all participants are informed that, if they have any complaint regarding the manner in which a research project is conducted, it may be given to the researcher or, alternatively to the Secretary, Human Research Ethics Committee, Registrar’s Office, University of Western Australia, 35 Stirling Highway, Crawley, WA 6009 (telephone number +61 8 9380-3703). All study participants will be provided with a copy of the Information Sheet and Consent Form for their personal records.
